Enhanced Care Management, Delivered by People Who Have Been There
Restorative Community Solutions is a contracted Enhanced Care Management provider with Partnership HealthPlan of California, serving justice-involved adults and youth and people experiencing homelessness in Solano County.
We are accepting referrals now. There is no cost to members.

About the program
Enhanced Care Management (ECM) is a Medi-Cal benefit for people with complex health and life circumstances. Every member gets a single Lead Care Manager, one consistent person who helps coordinate medical care, mental health and substance use treatment, housing, benefits, transportation, and the everyday logistics that decide whether a reentry plan holds together.

RCS provides ECM for people returning to Solano County from jail or prison, for people released within the past twelve months, and for adults and youth experiencing homelessness. We meet members where they are: at home, in the community, at an encampment, at a doctor’s office, or wherever works. Services are voluntary and free to eligible Medi-Cal members.

Why RCS?
RCS is peer-led and community-rooted. Our organization was founded and is run by people who have lived the reentry experience in this community. We are not a call center and we are not a distant institution. When our care managers say they understand what the first ninety days home feel like, they mean it.​
We built this program to prove something: that community-born, locally governed organizations can hold Medi-Cal contracts directly and deliver care that reaches people large systems miss. Our contract with Partnership HealthPlan of California is that proof, and our members are the reason for it.



What a Lead Care Manager does

Builds a care plan around the member’s own goals

Coordinates doctors, behavioral health, and dental care so nothing falls through the cracks

Connects members to housing help, food benefits, transportation, and Community Supports

Supports the transition from custody, hospital, or crisis back into stable daily life

Shows up: in person, in the community, on the member’s schedule

Who is eligible
Adults and youth in Solano County who are Partnership HealthPlan of California Medi-Cal members and who are:

Transitioning back to the community from incarceration, or released within the past twelve months, or​

Experiencing homelessness


How to refer
Anyone can refer: probation and parole, jail and prison reentry staff, hospitals and clinics, behavioral health providers, community organizations, family members, or the member themselves.
